Contact Surveys
If the survey is not related to a program activity, you can also create survey invites in the Contact Management section of the database. These surveys will be linked to the contact record only and will not be associated with mentoring or case management.

Go to the Contact—Individual screen.

Pull up the record of the contact.

Click on the Survey tab on the bottom portion of the screen.

Click on the Add New button at the bottom of the screen.

In the Survey Invite box, select the appropriate survey from the drop down list, the appropriate response group, and enter an invite expiration date.

Modify the default email, if needed.

Click Save & Send Email if the recipient will be receiving the survey via email. Click Save if you will be taking the survey on behalf of the contact.
