2 forms and 2 surveys come with the Premium Package.
“Forms” will create a Mentor or Mentee record and the information from your applications will be mapped into fields in your database:
The “Surveys” are individual links that you would send out from the database after the Mentee, or Mentor already has a record in the database. They do not map into fields like the forms do, but you’d collect the information for aggregated reporting. You will see the results to your surveys here:
We can certainly add and map additional forms, or surveys for you, but typically there is a $500 charge for any additional forms and $250 for additional surveys.
